Academy membership materials

CV Guide for promotion in educator track

Academies may have varying membership structures. Some academies, such as the Harvard Medical School Academy, have a one-level system where members are not distinguished by seniority. At the HMS Academy, members apply for two-year terms, and they can renew their membership every two years if they meet the requirements for membership. ​Other academies have three-level systems, such as the University of Utah and Duke University, where there is a distinguished level, member level, and then student/trainee level. 

Harvard Medical School, University of Utah, and Duke University have all kindly shared materials for general use. Please feel free to modify the forms and applications as needed.
